Another "lurker" asking a question.

 This is not specifically a sedation question but would appreciate
anyone's experience related to Port catheters.  My group of three PICU
docs provide deep sedation for the interventional radiologists to place
ports primarily for peds onc patients but also peds neuro patients who
need frequent IV access.  We have had issues with 3-4 catheters becoming
seemingly brittle and requiring additional care with removal.  E.g. one
sent to the OR and one breaking apart with asymptomatic pulmonary
embolization.  The catheters are fine during use and the interventional
radiologist are using wires and snares to assure safe and complete
removal.  The manufacturer and the FDA are involved and no recall has
been issued.  I would be interested to know if anyone has had a similar
experience.  We are unsure if the catheter issues are related to age
(greater than 2 yrs of use), exposure to particular chemo agents, a bad
lot, etc.  Without good data we are guessing as to best patient care
with either pre-emptive removal/replacement (which is when the issues
occur) vs. watchful waiting.  The manufacturer and FDA are not able to
share their data until investigation is complete. The radiologist do not
have a similar listserve to share experience thus the use or misuse of
this listserve.  We would appreciate any thoughts.
